- RULES AND REGULATIONS OF COLLEGE
Following instructions are to be followed as regards charges for the treatment of physiotherapy students:
a) OPD
• OPD Card: Free
• All Laboratory and Radiological Investigations: 30% Discount
• MRI and CT scan: 30% Discount.
b) IPD
• Admission Charges: Free
• All Laboratory and Radiological Investigations: 30% Discount
• All other charges (Bed charges, Procedure charges, diet etc): 30% Discount
c) Pharmacy, Dental Services, Blood Centre Services, Cosmetic Services, outsourced laboratory investigations, implant charges are not included.
d) The above instructions will only be applicable to students under treatment from faculty of Adesh Hospital.
e) No outside Prescriptions/Case reports will be honored.
- LIST OF ANNEXURES /AFFIDAVITS TO BE SUBMITTED BY ALL CANDIDATES AT THE TIME OF ADMISSION TO THE COLLEGE:
i. Enrollment form for admission in B.P.T.
ii. Self-undertaking of gap in study, if any.
iii. Undertaking by parents/guardian.
iv. Joint undertaking by the candidate and parents/guardian.
v. Joint undertaking regarding indiscipline.
vi. Request for hostel accommodation.
vii. Affidavit by student regarding ragging.
viii. Affidavit by parent/guardian regarding ragging.
